- AT2 antagonist
Action
- There are two Angiotensin II receptors (AT1 and AT2)
- AT1 receptor effect are vasoconstriction and aldosterone release
- Angiotensin II receptor blocker and so blocks the effect of AGII
- Causes vasodilation and a drop in BP
- Losartan is a prodrug and concerted to the active form by the liver
Indications/Dose
- Hypertension, Heart failure, Treat and prevent diabetic nephropathy
- Losartan 25-50 mg po initially increased slowly to a maximum dose of 100 mg od po
Dose range: Check U+E, BP, target BP. Check BNF

Contraindications
- Pregnancy - cause birth defects
- Caution in severe aortic stenosis and hypertrophic cardiomyopathy
Interactions
- May be used with or instead of ACEI
Side effects
- Hyperkalaemia (caution with potassium sparing diuretics)
- Not associated with cough unlike ACEI
Monitoring
- Check renal function 4 days and 2 weeks following initiation
- Check renal function 1 week after any increase in dose
