Many cases in the past occurred 2 to 3 weeks after a Group A Beta-haemolytic streptococcal sore throat with the onset of smoky urine due to haematuria and signs of uraemia. Most were transient with full recovery. Seen less so now with widespread antibiotic usage. Likely to have been immune-mediated.
