Indications/Dose
Name Starting Dose Frequency Route Fondaparinux (VTE prophylaxis) 2.5 mg OD (Often at 6pm) SC Fondaparinux (ACS - USA/NSTEMI) 2.5 mg OD SC. Treat < 8 days . Stop before CABG and restart after 48 hrs Fondaparinux (ACS - STEMI) 2.5 mg IV for the first day, then 2.5 mg SC Treat up to 8 days OD First dose IV then SC < 8 days . Stop before CABG and restart after 48 hrs Fondaparinux (Acute DVT/PE) 5 mg (body weight < 50 kg)
7.5 mg (body weight 50 to 100 kg)
10 mg (body weight > 100 kg)OD SC. Fondaparinux should be continued for at least 5 days and until INR > 2 for at least 24 hours Fondaparinux (Superficial-vein thrombosis) mg SC Interactions
