A negative tissue biopsy does not rule out the diagnosis of amyloidosis as it may be patchy except for the heart, where the disease is usually widespread if there are echo changes

Primary Amyloidosis (AL) | Amyloid Light chain Ig. Myeloma, Waldenstrom's, MGUS, Lymphoma - Cardiac/Renal as above, Periorbital purpura. AL Amyloid results in Restrictive cardiomyopathy and Macroglossia. Development of amyloid in myeloma is associated with a worse prognosis |
Secondary Amyloidosis (AA) | Amyloid Serum Amyloid A protein - Chronic Inflammation e.g. Bronchiectasis, Osteomyelitis, TB, Familial Mediterranean fever and rheumatoid arthritis, TB, Osteomyelitis possibly due to Serum Amyloid A protein and patients develop Nephrotic syndrome Very rarely cardiac/tongue. AA Amyloid very rarely results in Restrictive cardiomyopathy and Macroglossia and these are more typical of AL amyloid |
Familial (ATTR) | Amyloid Transthyretin - Hereditary forms of Amyloid - sensory/autonomic neuropathy with renal/cardiac involvement |
Beta 2 Microglobulin. Family history of amyloid a neuropathy may suggest ATTR | renal dialysis and carpal tunnel syndrome |
