There are several mechanisms through which drugs can cause patterns of liver damage. The liver problems may just be a slight increase in transaminases which requires no action or acute liver failure. They rarely cause chronic disease.
Side Effect | Associated Drugs |
---|---|
Cholestasis | Chlorpromazine, which can induce a hypersensitivity reaction leading to cholestasis in about 1% of patients, typically within the first four weeks of therapy. High-dose oral contraceptive pills (OCPs) can also cause cholestasis. |
Mixed Hepatic/Cholestatic Hepatitis | Nonsteroidal anti-inflammatory drugs (NSAIDs), Co-Amoxiclav, and Statins are known to cause a combination of hepatocellular and cholestatic liver damage. |
Acute Hepatitis | Anti-tuberculous medications such as Isoniazid and Rifampicin, Paracetamol (in overdose), Methyldopa, Atenolol, and Halothane can all lead to acute hepatic inflammation. |
Non-Alcoholic Steatohepatitis (NASH) | Amiodarone, Tamoxifen, Tetracyclines, and Sodium Valproate are drugs that may lead to fat accumulation in the liver, progressing to inflammation and fibrosis, a condition known as NASH. |
Venous Obstruction (Veno-Occlusive Disease) | Busulfan and Azathioprine can cause obstruction of small hepatic veins, leading to liver congestion and damage. |
Hepatic Fibrosis | Methotrexate, particularly with long-term use, is notably associated with the development of hepatic fibrosis. |
